To all parents/carers of children and young people who have a health care plan in place for school. The school Nursing service will write out to each home address over the summer to review your child/young person's care plan for the upcoming year in school, if you have moved home in the last year, it is important that you let us know. You can do this by phoning
02828261951 and leaving a message detailing your child's name, DOB, school attended and your new address.
For those young people moving to post primary schools, please ensure that care plans are requested from your current school and provided to the relevant post primary school on or before the first day. For those children and young people who have emergency medication in school, please ensure that you have a supply of in-date medication ready for the first day of school starting back in Aug/Sept.
